Take part in the evaluation for the WWDA LEAD Project!

The WWDA LEAD Project is due to end in June 2024 and the WWDA LEAD Project team are starting to gather feedback from participants that took part in different activities and events across the LEAD Project. If you took part in part of the WWDA LEAD Project, we would love to hear from you.

Confidentiality and Participation

Responses will remain confidential. All answers and information you provide us will be kept securely, in accordance with WWDA’s Privacy Policy. You can close the survey at any time you if you don’t want to continue.

You do not have to answer every question in the survey if you don’t want to.

We want to make sure you know that your data is private – we won’t be able to trace it to you if you don’t want us to. You are under no pressure to contribute – and if you decide not to contribute it will not affect your relationship with WWDA. We’re going to use your thoughts to help us reflect on WWDA LEAD, improve our work in the future, and report to our funder and members. 

Eligibility

To participate in this crucial part of the LEAD Project you must have participated in at least one activity or event throughout the course of the LEAD Project.

Participation in the WWDA LEAD Monitoring, Evaluation and Learning closes on the 1st of June 2024.

Ways participate in the WWDA LEAD Monitoring, Evaluation and Learning

There are two different consent forms that are linked below. You can take part in both, one or neither.

Survey Consent Form

Survey Consent Form Part 1 – Easy Read

Survey Consent Form Part 2 – Easy Read

This form will ask questions about you and about how you feel your leadership has developed as a result of the LEAD Project.

Timing

There are 33 questions within this form. There are two (2) consent questions at the beginning and two (2) questions at the end that require a response. All other questions are optional. This form should take around 15-20 minutes to complete. Participation in the WWDA LEAD Monitoring, Evaluation and Learning closes on the 1st of June 2024.

Change Story Consent Form

Change Story Consent Form – Easy Read

This form will ask questions about you and then will ask you for a different way you might like to participate in the monitoring, evaluation and learning for the LEAD Project. There are many options you can choose from such as a written self-reflection, poem, or take part in group discussions.

Timing

There are 18 questions within this form. There are seven (7) consent questions at the beginning and two (2) questions at the end that require a response. All other questions are optional. This form should take around 10-15 minutes to complete. Participation in the WWDA LEAD Monitoring, Evaluation and Learning closes on the 1st of June 2024.
